Her active participation in all National Health Programmes under National Health Mission (NHM) is highly appreciated.

 She has to face several challenges while working in a rural area with high illiteracy rate, lack of health care facilities and staffing that affect effective delivery of health care. In the midst of these challenges, her performance in all areas of her work remained excellent.

Smt. Savitaben L. Chauhan is a best performer in all the State and National Health Programmes. Her involvement in cholera disease surveillance in Dudhri and Khanvel area in 1986, detection of plague at Bavisa Faliya in 1994 and floods in Silvassa in 2004 are highly commendable. She had attended several State and National level training programmes to update her knowledge and skill. Over three decades of her service in a remote area, she has exhibited high level of devotion, commitment and sincerity.

 She had received two best performance awards for her outstanding work related to annual reporting of registration of births and deaths in her Centre.

Smt. Bhagyalaxmi K. Solanki has more than two decades of experience in sub-centre as well as primary health centre at Ghoghla, Diu. She actively participates in implementing all State and National Health Programmes such as Pulse Polio Programme (1995), National Leprosy Control Programme (2001-2005), Measles Rubella Vaccination Campaign (2017), School Health Programmes, RCH and Family Planning.

 During her work she achieved 90% polio immunization coverage. She has also vaccinated 12,836 children against Measles Rubella and achieved 100% coverage. Her involvement in 2001 cyclone in Saurashtra, particularly in schools of Bucharwada area was highly appreciated.

She is known for her dedication and sincerity. Besides her responsibility at PHC, she had additional position as ANM incharge at CHC, Ghoghla, Diu.

Smt. S. Ashia has been working for thirty years in the Sub-Centre which covers Shajouba village, which is a hilly and tribal area. Her maternal and child care services to the community are commendable. Her selfless and dedicated services to the public are highly valued and appreciated. She reaches people by foot, to render services, 13-16 km away from the Sub-Centre. She has conducted 300 home deliveries in the interior and inaccessible villages to meet the emergency needs.

 Beyond her duty she also conducts health camps and provides food and care to the elderly.

She was appreciated and awarded for the outstanding co-operation to the district administration and commendable service to the public health. She is a certified health worker to practice IUCD insertion. She has updated her knowledge and skill by attending various State and local level workshops.

Smt. Anita Kumari Parida is instrumental in achieving 100% institutional delivery, 85% antenatal care and 74% immunization coverage in her Sub-Centre area. Her outstanding performance in the field activity in a most economically backward tribal district of Odisha is commendable.

 She is a disciplined and committed health worker. She is awarded as the best health worker (female) in the year 2018 at the district level. Her involvement with the community in the Japanese Encephalitis (JE) outbreak in 2016 was exceptional.

Smt. B. Vijaya Laxmi works as an Auxiliary Nurse Midwife (ANM) at Kalabgur Sub-Centre in Kandi PHC. Over her twenty five years of experience, she has demonstrated passion and dedication in her work. She responds to needs of all people immediately without hesitation even beyond her duty.

 She is a good ambassador for health activities in the service area. She has excellent counselling capabilities and UNICEF has chosen her as ambassador on their health calendar released in 2001. She submits facility based reports with 100% accuracy. She has updated herself by undergoing several workshops and was identified as master trainer to inculcate the MAARPU concept.

 In Kalabgur service area, the deliveries in government facilities were more than 75% whereas in the district, it was only 41% in 2013. She is credited with starting one of the first centres for counselling of adolescent girls by conducting regular meeting and addressing issues faced by adolescent girls.

She is one of the best health care promoters for government health services, thereby reducing unnecessary financial burden for the rural poor. She has received several district awards as the best Auxiliary Nurse Midwife (ANM).

Smt. Shima Bhowmik in her twenty years of experience in various sub-centers and currently in AGMC & GBP Hospital Agartala, has demonstrated excellent performance in her work. With special talent of counseling and motivation, she is rendering counseling to antenatal and postnatal mothers on Mother-Child Care in the AGMC & GBP Hospital. Her involvement in immunization, health awareness programmes and all State and National Health Programmes is commendable.

As a member of an NGO and Red Cross Society, she is engaged in health related activities during her holidays. Her involvement with tribal community and rural schools during outbreaks of malaria and cholera and health awareness programmes is highly appreciated.

She has obtained Diploma in Homeopathic Medicine as an additional qualification and she utilizes this knowledge in providing primary care to the rural community.

She serves the rural community with integrity, dedication and discipline. Her selfless and sincere efforts in providing primary care to tribal people living in hilly regions are highly beneficial.

Working from a Sub-Centre situated in a hilly and tribal area of Jalpaiguri, Smt. Sonali was able to promote and achieve 100% institutional delivery during the year 2017-18. In two situations, she had saved the lives of mother and child during child birth. She had conducted several health awareness programmes on mother and child care, girls’ trafficking, avoidance of early marriage, sex education and menstrual hygiene in collaboration with an NGO ‘Child in Need Institute’, North Bengal Unit, besides her regular service at the Sub-Centre.

Her involvement in community development through health awareness programmes was phenomenal. She is highly proficient in her work and she actively participated in implementing all State and National Health Programmes.

Smt. Tage Munya has served as Auxiliary Nurse Midwife (ANM) and Lady Health Visitor (LHV) in General Hospital, Naharlagun, Arunachal Pradesh for 16 years and later as Lady Health Visitor (LHV) in Balijan Community Health Centre (CHC) under Papum-Pare district and Urban Health Centre Itafort. She has demonstrated full dedication and sincerity over the twenty three years of service with the Government of Arunachal Pradesh. While at Community Health Centre (CHC), she had to cover remote areas on foot around 25 km/ day during implementation of the State and National Health Programmes.

 Her exceptional knowledge in expanded programme of immunization and leadership skills to work in co-ordination with other departments has helped in very good service coverage. She had vaccinated 20,000 children in the recent national programme of Measles Rubella Vaccination, which is a remarkable service appreciated by the state authorities.

 She is innovative as well as quick in crisis management as per the need of the situation.

Smt. Ila Chutia as Auxiliary Nurse Midwife (ANM) had served Sub-Centre under Kamalabari Block Primary Health Centre and later as Lady Health Visitor (LHV) in Primary Health Centre (PHC) with 34 years of total experience. Her work was mainly in a remote village within an Island district which was termed as flood prone. She has served the people during floods reaching them by walking or in boat. Her main areas of service included Maternal and Child Health (MCH), health awareness and school health programmes. She actively participated in implementation of all National Health Programmes.

 Beyond her duty, joining with an NGO “IMPACT NE” she provided extraordinary voluntary services to leprosy patients and patients with HIV/AIDS, diabetes and hypertension, particularly in the areas of health education and counselling.

As a Lady Health Visitor (LHV), her performance is outstanding. She is a hardworking, sincere health worker with adequate knowledge on various health related topics. Her services in a flood affected remote area of Assam is praiseworthy.

Following promotion as Lady Health Visitor (LHV), she now works at KG Hospital and LAMM College, Raigarh in the department of immunization. Her special contributions towards organizing eye camps, management of biomedical waste, immunization and child care are noteworthy.

 She is a dedicated and sincere health worker with leadership abilities. She takes independent decisions during emergency situations within her limitations.

Pant Hospital, before her retirement in 2017. As a staff nurse and officer, she has contributed significantly to the welfare of her patients in OPD and casualty. She has provided excellent care to her patients with high level of dedication and sincerity. She was always willing to escort referred patients and VVIPs. She had worked long hours beyond her duty timings and had always risen to occasions in times of need.

 Her contributions during Tsunami/Earthqauke and Vardah Cyclone (2016) were highly appreciated. She worked for more than 24 hours at the emergency unit. She was incharge of disaster team and she effectively managed disaster situations.

 She has organized many blood donation camps and encouraged many doctors and nurses to donate blood. Even after retirement she has volunteered to work in OPD/casualty department in the same hospital.

 She was awarded ‘best nurse’ in conducting deliveries at Primary Health Center (PHC), Machavaram at the district level. She has exhibited keen interest in updating her knowledge and skills by attending training programs on HIV/AIDS Counseling, F-IMNCI Training, Training of ANMs by GFATM, SBA Training, and AIDS Awareness Programmes conducted at local, state and national levels.

She manages operation theatres effectively and has set up very good protocols particularly for infection control practices in operation theatres. In a fire accident case, she has saved the lives of 12 patients in recovery room with the help of another health care team member.

 She demonstrated keen interest in updating her knowledge and skill by attending various training programmes conducted at local, state and national levels. She is a member of Trained Nurses Association of India.

 She has worked as a volunteer with Regional Organ and Tissue Transplant Organization (ROTTO), PGIMER, Chandigarh, to promote the altruistic cause of organ donation. Her commitment to this cause speaks about her spirit of social responsibility. Working beyond her professional duties, she has been able to inspire 175 people to sign up for organ donation including herself and her daughter. (Nursing) qualification, she has also done M.A. (Psychology), Advanced Diploma in Child Guidance & Counselling and Certificate in Computer. She is an excellent counselor and her advanced education enabled her to excel in her counselling skills.

 Her major areas of work include annual medical checkup, screening of students, cumulative health profile, health education, follow-up, emergency services and counselling. In all these areas her performance is extraordinary.

Her extensive survey of J.J. Clusters in Delhi during the cholera epidemic in 1988 in East Delhi, had led to setting up of a mobile health scheme, to which she has contributed significantly. The services include OPD, referral and health education. She has organized and worked in special health checkup programmes, mass deworming for school children, health melas, national health programmes and health rallies. She has excellent team spirit and works with integrity.

Her dedication in handling difficult and emergency situations is commendable. She is a creative and innovative person with a proactive approach to her work. She has excellent management skills and knowledge, and was awarded at state level for her meritorious services.

Shri Sushant S. Bhatikar with over 20 years of experience has worked in dialysis unit for 13 years and currently works as diabetes educator in Goa Medical College & Hospital. He is the first diabetes nurse educator in Endocrine Department of Goa Medical College & Hospital (2010). His interest in patient education and commitment has enabled him to choose this job. He attended training programmes at various institutes in the country and abroad namely Diabetic Foot Project India-Germany 2010, Project Padsamrakshane II at Jain Institute of Vascular Sciences, Bengaluru and National Diabetes Educator Programme. These trainings enhanced his knowledge and skill in dealing with diabetes education and foot care.

 Besides attending training programmes, he has conducted several workshops for physicians and nurses and is involved in giving lectures on diabetes self-care. He is also involved in multicenter trials over the last few years.

He is humble, committed and passionate in his work as diabetes educator. He is very approachable and has good rapport with his colleagues and public.

His work as a staff nurse at Community Health Centre Danta, a tribal area and ‘Jivan Raksha’ project on a highway working continuously for 72 hours were highly challenging. He worked with dedication and sincerity. While working at Community Health Centre Kamrej, Gujarat, he has conducted more than 500 normal deliveries.

 His contribution during natural calamities such as earthquake, Surat Flood (2006) and Flood at Banaskantha (2015) is commendable. He has also conducted a normal delivery in a running train. Through these acts, he has demonstrated his sense of social responsibility.

 He has served as infection control nurse in the S.S.G. Hospital for three years. He has attended many workshops and conferences at state and national levels. He has been a member of disaster management team. He has received many appreciation letters from local authorities for his input in various natural calamities. She organizes and conducts classes for unit housekeeping staff that contribute towards improving quality of nursing service.

She also provides voluntary service to the community sewa samitis. She has received best nurse award in 2017 by TNAI Kerala State Branch.

Shri Akbar Ali P.K. has a decade of experience in hospital, CHC Amini Island and on Ship MV Kavaratti (Coastal Voyage). Currently he is working on board vessel MV Minicoy. He is a dedicated, hardworking and sincere staff. His courage, professionalism and patriotism had enabled him to join the rescue mission ‘Operation Rahat’. His active participation and contribution in the mission in 2015 had led to the successful evacuation of distressed and stranded citizens of India and foreign nationals from Bangladesh and Yemen which became a national pride. He has high level of commitment, perseverance and devotion to duty. He was part of the team (Indian Navy), which rescued 350 Indians.

 The nursing care provided to a Yemeni patient receiving dialysis on board and conduction of delivery of a passenger on board by him are noteworthy. All these events were displayed by media.

 He was awarded certificate of recognition for the outstanding service rendered by him on board, joining the ‘Operation Rahat’. His tremendous bravery and determination in undertaking this mission is appreciated.

As a Public Health Nurse in Community Health Centre (CHC) for the past six years, she had demonstrated excellent performance in all areas particularly in Maternal and Child Health (MCH) services. She was appreciated for bringing down Infant Mortality Rate (IMR) with no Maternal Mortality Rate (MMR). Her special interest is on adolescent health and has prepared patient education materials on adolescent health, suicide and breast cancer. She is trained as a master trainer in non-communicable disease and adolescent health. She works with dedication and sincerity. She has conducted several health awareness programmes for the community and school students. She was awarded the best Public Health Nurse (PHN) award by the State of Puducherry.

 She had continuously updated her knowledge and skills by attending various training programmes at national, state and local level. She has served as resource person to deliver lecture on adolescent health. She has also paper presentations and publications to her credit. She has organized many programmes on celebration of special days such as Breast Feeding Week, World Mental Health Day and World Suicide Prevention Day. She is a member of professional organizations namely TNAI and Society of Midwives India.

She has high sense of social responsibility that is evident in her adopting a village for upliftment, organization of blood donation camps and work among gypsy community. Her contribution during floods by working continuously for 15 days and rescuing a child form near drowning were appreciated by state authorities.

Ms. Rupa Upadhye is working in J.W Global Hospital & Research Centre, Mount Abu on honorary basis for the past 26 years after leaving government service at Mumbai. She has worked in various clinical areas of the hospital and currently as chief of nursing services. She was instrumental in setting up a clinical skill lab at J.W. Global Hospital to train doctors, nurses and nursing students on CPR, IV cannulation, intubation and nursing procedures. She has undergone a simulation training programme for clinical skills in 2015. She has updated her knowledge and skills by attending, organizing and serving as resource person in several state and national level workshops.

 She is the first female nurse in India for High Altitude Medical Rescues Operations conducted by Indo Tibetan Border Police Force and Six Sigma Altitude Medical Services, 2016 Delhi. She has done rescue operations and saved 5600 victims under hazardous conditions like Uttarakhand Flood (2013) and Nepal Earthquake (2015).

 She is always at the forefront in organizing and sending nursing teams to disaster stricken sites. Her contribution in a mega project titled ‘My India Healthy India’ is highly significant. It is a health awareness campaign conducted by medical wing of Rajayoga Education & Research Foundation in which 3.50 lakhs of people participated in covering a distance of 20,000 kms that ended in Mumbai.

 She has participated in numerous health rallies, hosted health fairs and health awareness campaigns to educate masses on healthy lifestyle, personal hygiene and nutrition. She has received national award for ‘best nurse of the year 2017’ by Six Sigma Healthcare excellence.

 Her passion, fearless and selfless services for the overall betterment of nursing and the people of India are highly commendable.

For the past 8 years, she is primarily engaged in receiving Sikkim government referred, terminally ill cancer patients and assisting them in getting appointments and treatment in AIIMS, Apollo, Medanta, ILBS, Rajiv Gandhi Cancer Hospital and Indian Spine and Injury Centre. She visits various hospitals accompanying patients, assists them in language translation. She takes care of patients in D.K.K. Bhawan during emergencies and helps patients during death, making arrangements for transport to Sikkim or cremation in Delhi. Her contributions in providing emergency care to terminally ill patients with compassion and caring attitude are highly significant in such challenging situations.

She has actively participated in various short training programmes for nurses conducted by AIIMS, Apollo and Rajiv Gandhi Cancer Hospital. She has undergone Diploma in Oncology Course in Tata Memorial Hospital.

Besides taking care of terminally ill cancer patients from Sikkim, she also visits institutions for old aged people and women & children to give them health talks on various diseases and its prevention. She also works with some NGOs for prevention of human trafficking. Her work for critically ill, providing palliative care, assisting doctors in natural calamities, accidents, surgery camps for reconstructive surgeries of burns and acid attacks are highly laudable.

Her extraordinary contributions towards Tribal Health Programme for more than 10 years are highly appreciated and awarded by Health Secretary of Tamilnadu. Besides her regular routine hospital work, she has organized and participated in many tribal health activities such as health awareness campaigns, dental and eye camps, cancer detection camp, mother care camp and many other camps. Through her services, the mother mortality rate (MMR) and infant mortality rate (IMR) were brought down and institutional deliveries were promoted. As a part of mobile tribal outreach programme, she has provided medical services to people in hilly areas such as Kolli hills, Yercaud and Arunuthu hills.

 Her contribution during Chennai and Cuddalore floods and blast accident in Tamilnadu Magnesite Ltd. in providing emergency and first aid to needy people were appreciated.

In another incident in orthopedic operation theatre, she identified hypotension in a patient and was instrumental to initiate interventions to save the patient. She is excellent in maintaining records at her work place.

She has been appreciated for her distinguished services as administrator by TNAI, Uttarakhand branch in 2007. She has been conferred certificate of excellence by India International Friendship Society in 2012.

She has additional qualification in Mass Communication also.

As a Manager of ICU, she introduced modernization in ICU care. She prepared various nursing protocols and using videos she trained nursing staff to implement those protocols. Her managerial skills in management of ICU and staff motivation and guidance are highly commendable. Her exemplary care to critically ill patients contributed significantly to successful recovery. She is creative and innovative. Through her efforts, she developed a system of students from other institutions to have clinical training in Central Hospital benefiting the hospital, students and patients.

She volunteered services to help the injured passengers at the accident site following the derailment of Kalinga Utkal Express on August 19, 2017, during her sanctioned leave. She stayed whole night at the first aid post helping the injured passengers to minimize their pain and grief. She was also instrumental to organize a vacant surgical ICU functional to accommodate the injured passengers following derailment of Howrah-New Delhi Rajdhani Express on September 9, 2002, which caused 119 deaths. She had received appreciation from Northern Railways.

 She is an excellent bed side ICU nurse, organizer and manager.
